After we were done my ped informs me that I need to sign some form that says a bunch of stuff about that he has gone over with me about shots and that I've chosen not to have shots done and blah blah blah. Anyways,I didn't sign the form..I said I would like to take it home to have my husband review this before I sign. Have any of you come into contact with this form and what have you all done?
