Refusal to vax form question...

I apologize for a repeat if this has been addressed already, but what can I do

if I already signed that form about choosing not to vax when my daughter was

first born. She is nearly four years old now. Is it possible to ask for it and

change it right there in front of them? Would they even let me see the original?
