What if I already signed it? When my son was 6 weeks old (he is now almost 2) I

signed it not knowing not to. I did cross out the section stating I was

accepting resp. for endangering my son and I wrote next to it " I do not agree

with this statement " . We have not been back to the doctor since and we now see

natural doctors for well visits. I have Kaiser so this form will always be in

my sons folder and I get calls all the time from Kaiser (about 2 a month)

stating that my children are not up do date on their immunizations and I need to

make an appt. to bring them up to date. " Of course I just delete, however I am

a little apprehensive that if we need to go to the doctor or hospital for some

reason they will see the form. Also it comes up on the registration computer

that my children are NOT VACCINATED At ALL. I get a printout of the form as a

receipt and all of the shots are in list order with NOT RECEIVED/OUT OF DATE

next to it. I could not lie if I wanted to. I

worry what they write in my charts because the last time we were there (when my

daughter had MRSA) they were pushing me to bring her up to date on her boosters

(no vax since age 4 and she is now 8) and get on antibiotics. I refused and we

treated her with Homeopathy and boosting her immune system. Thankfully I never

needed antibiotics but the doctor made a comment like " From her chart I can see

that you like to do things naturally. " I had never seen this doctor before and

was curious how she knew that. There must be something in my childrens charts

that say what my medical beliefs are. A little unnerving. Sorry for the long

email...just venting a little too!

* Write your own form.

* Use our form 'as is' or modify it to reflect your needs.

I have signed it everytime I've gone in to see our ped. YIKES! I like

the other info she gives me, very AP and pro breastfeeding so I

wouldn't want to change Drs. However, the other doctor she works with

I DO NOT LIKE AT ALL!

Anyways, what do I do if I've already signed it, not once, not twice,

but probably about 10 times? My son is almost 2.

You could always try to get them back, but you will likely have to change

doctors
