Some states make it easier for you; for some you might need to get a lawyer. CT has religious and medical exemption forms available online at the DPH website. I use them with no problems at private and public schools, camps, too (although technically they shouldn't need them since the law doesn't apply to camps). Plus, at least here, now there is even a spot on the standard health form to check off if you have an exemption. I would be very careful about adhering to the legal language, and then keep my mouth shut if asked any questions about my religion (never been asked, by the way). Guest guest Posted January 16, 2008 Report Share Posted January 16, 2008 Here's statute 1003.22 from the FL DPH website (#5-a has exemption info) under A-Z topics: http://www.flsenate.gov/Statutes/index.cfm?App_mode=Display_Statute & Search_Strin\ g= & URL=Ch1003/Sec22.HTM Then this link has the specific info (pg. 15-16) on form DH-681 needed for a religious exemption, although I don't see a link to get the form online: http://www.immunizeflorida.org/schoolguide.pdf Also, http://www.immunizeflorida.org/qi_clinical/faq.htm: " A Religious Exemption From Immunization (DH form 681) can be issued if immunizations are in conflict with the religious tenets and practices of the child's parent or guardian. This exemption can be issued only by a county health department and only based on established religious beliefs or practices. " So it seems you need to request the form from your county health dept. You look at the law for your state and use the exact wording of the law. Some states are a little more difficult such as NY, IL and maybe NJ. Which is really fine in the long run.
