No, you can't just take a form off the internet. Each state has specific

language that needs to be addressed. Some states make it easier for you; for

some you might need to get a lawyer.

CT has religious and medical exemption forms available online at the DPH

website. I use them with no problems at private and public schools, camps, too

(although technically they shouldn't need them since the law doesn't apply to

camps). Plus, at least here, now there is even a spot on the standard health

form to check off if you have an exemption.

I would be very careful about adhering to the legal language, and then keep my

mouth shut if asked any questions about my religion (never been asked, by the

way).

I am from Florida. You don't need it. Email me privately for more info. I have

kids 6 and 4 who had waivers and entered school - no issue or controversies came

my way. And I dealt with this just yesterday in my new home state of Nevada.

Here's statute 1003.22 from the FL DPH website (#5-a has exemption info) under

A-Z topics:

Then this link has the specific info (pg. 15-16) on form DH-681 needed for a

religious exemption, although I don't see a link to get the form online:

Also, http://www.immunizeflorida.org/qi_clinical/faq.htm: " A Religious Exemption

From Immunization (DH form 681) can be issued if immunizations are in conflict

with the religious tenets and practices of the child's parent or guardian. This

exemption can be issued only by a county health department and only based on

established religious beliefs or practices. "

So it seems you need to request the form from your county health dept. Hope this

No, sorry, this is not how you do it.

You look at the law for your state and use the exact wording of the law.

Some states are a little more difficult such as NY, IL and maybe NJ.

Yes, it is a big deal.

Most pediatricians will not accept that and will throw you out. Which is

really fine in the long run. If you don't want their vaxs you soon figure

out you don't want much else they offer.

It is just not that easy...............

Once you have worked in this area as long as many of us have, you will see.

Yes, Florida is easy! Just go to your local health Department and

request the blue relgious exemption form. Piece of cake!
