Hi Dave,

I have a good informed consent, covers all the legal stuff and

NF/biofeedback/counseling. I am more than happy to share it with you...let me

know if you want a copy...I can fax it or email it to you.

Dave,

I had one on my Mac that I used for years before we finally dropped it. Unfortunately it was lost when my Mac died (perhaps of heat exhaustion) during my move from Atlanta to Miami.

Hopefully others on the group--maybe Rosemary or Rosemary--have something they could share.

Hi Dave,

I only have one informed consent and it covers all the work I do, so you can

look it over and take what you need. Becaause I have a license it covers all

bases especially the neurofeedback stuff. It is also my policy form and has

my prices, with cancellation policy as well. I will email it on Monday when I

am back at my office.

version is really not very good.

I am not surprised. That was a reason I didn't want to have my first at the

hospital. But,

as a home birth didn't work out for me, we ended up at the hospital. I had my

husband

stay w/the baby at all times and it was in our birth plan - no vitamin k, no

hepatitis b, and

no eye goop. I think the hospitals do certain things out of routine for all

children, so you

have to watch them so that they don't do something you don't want just out of

habit.

That's my understanding anyway.

Sorry to say it happens ALL the time.

Its kind of an opt out thing

If you don't opt out it is part of the care.

Risk you take when you have a hospital birth

Someone needs to be with your baby 24/7 - someone you know and trust

But first you have to even be aware that there is a risk

i am not sure they asked me to sign anything either.I know when they

transferred his records to my ped they said they didn't send record that he

even had

that shot that i could call and see and if no records he could have it

again.I was like no he wont be either

didn't she have to sign some form of consent? They asked me to sign

a consent for my son...I didn't know what I was accepting...they

don't treat you like you will be denying it...they just say, here

sign for the hep b vaccine. This time...I wrote it on the birth

plan....No Hep B, No Vit K, no eye goop...
