Sorry for all of my queries. - When the en bloc explant 'removes the capsule', is that the silicone capsule of the implant or ??? I know a pocket was created behind my muscle for the implant but when I read that the capsule must come out, I thought the entire implant came out (capsule and gel/saline) - why not trans-axillary explant? If the patient wants the saline implant out, doesn't want a lift, why not a trans-axillary incision to drain the implant and pull out the capsule? I understand that it is important to remove any migrated silicone and the trans-axillary doesn't afford a whole lot of insight into the pocket to work...but what if an asymptomatic patient opted for explant via trans-axillary?
