Print this out and sign 2 places - vaccines and TB test (area under

the vaccine area has the TB test exemption)

Thank you Sheri,

I did sign the blue sheet. They called back saying either shots or no

school. After that I wrote a letter to ask for exempt for vaccine + TB

testing and quoted what California law says word to word.

Yesterday the school left a message saying the TB test has to be done

otherwise no school starting this Friday. This morning we handed over

another regarding TB testing (form found on this forum). So far nobody

called.

I have a question Sheri, I printed this out awhile back when putting my toddler

in her new daycare and signed it as normal to exempt her from any vaccines, now

because this card was not " given " to me for regular public school, is mine still

valid? I am in California!

I don't know - probably each school needs one.

Just print it out again and keep just in case they ask for it, it guess.
