Hi !

Do you remember checking off any boxes on the birth certificate

application? I would try to get a copy of the birth certificate

application and copies of all signed forms at your doctor's office.

Let us know what you did sign when you get them.

He was born in Arkansas and we moved to Texas 6 weeks later. I had to go through

some website called vital stats or something to get his birth certificate. I am

just feeling like I probably signed something at that first visit with this

doctor here in Texas when got that one shot. I was such a mess thinking about

even giving him the vaxes that I honestly shouldn't have even gone, much less

alone. I will call to get copies of everything though, that is a good idea.

Assuming I did give permission for the database, which I am sure I did, is that

like HIPPA where you can have it rescinded? Sorry if I keep asking a question

you aren't sure of.
