Yes - definitely put under no circumstances shall my child receive any

vaccines. Also, put your chiro's name unless you trust the no-vax

friendly doctor enough to treat your child (sounds like you don't like

to go there though?).

Yes, I would trust him to treat . I went to him faithfully for 2

years then sort of slacked off. I wasn't vaxxing and was never

sick. Once I started doing chiro regularly, I figured going to a ped on

a regular basis (unless needed) wasn't necessary.

I think I'll put both names.

Thanks for mentioning keeping copies - I always forget to do that!!

I put " except NO vaccines are to be given, not even tetanus. " Then initial it.

(Just did it tonight actually for a field trip permission form).

I would put both doc's numbers.

I also filled out one this week. I added, " with the exception of

immunizations of any sort, including the tetanus shot. " I think

someone posted something similar not too far back. (Thanks!!) I

didn't add our chiro & number. That's a good idea. We haven't seen a

ped since her 1yr visit, though we did see a family practitioner this

summer for the school physical. We regularly see a chiro and don't

vax so I don't see a reason to waste time/money to see a ped/fp (with

the exception of the school physical, of course, which I don't think

there's another way).
