Guest guest Posted September 16, 2006 Report Share Posted September 16, 2006 Hello JoAnn, Welcome, and I'm sorry you even had to look for us.......this knee business is no fun! You asked about unloading braces. I've had one since 2003 and it depends what your knee situation is on whether they work or not. I lost a " chunk " of cartilage on the medial condyle (the inside " knob " of the thigh bone) due to a bad fall. After a microfracture procedure (little holes poked in the bone to encourage a secondary type of cartilage to grow) my surgeon recommended that I wear an unloader brace to keep the full weight off that side of my knee. Bottom line, unloader braces only work if you have ONE side of your knee that is bad. The brace is a bit cumbersome, and it essentially pushes inward on the side that you don't have a problem, which then " opens " or " unloads " the side that does. It also keeps the knee from hyper extending. I no longer wear mine except for when I try to waterski, then I use it pretty much as a protective device. Mine was covered by insurance and is a custom fit, however they do make them " off the shelf " I believe...but getting a excellent fit is very important for you don't want it to slip. You must wear the brace next to your skin for maximum fit, so sometimes wearing pants like jeans becomes impossible. Some things to consider, getting a good physical therapist to work with you on strengthening your knee muscles (quads, hamstrings, hip muscles, etc) this will help alleviate the pain somewhat. It won't " cure " anything, but the trip to the store might be more doable. A lot of the exercises they give you are non-weight bearing so you won't have to be " in pain " to do them. Ask your doc to recommend someone and get insurance to cover it. If you don't have insurance, find a PT who will give you a complete home workout and then check in with them once a month. These knee strenghtening exercises are like brushing teeth........need to be done everyday to stay on top of things. I'm 48 and I understand your frustration about being essentially sidelined from life. I also understand why the doc is telling you to wait til your 60....... HOWEVER..........and this is a big one........TKR (total knee replacement) is an ELECTIVE surgery and depending on your disability, you can work at/with doctors to make something happen. The partial one, you're right knee, that is one that many surgeons will do because if/when it fails it's easier to " redo " it........the TKR, if it fails, is very difficult to replace again, which is why he/she wants you to wait. IT's a conundrum.......do you totally become an invalid while waiting til your 60, or do you have it done and hope that by the time the implant fails it can be redone. One of the reasons I was told that they don't like to do TKRs on younger people is that we demand a more active lifestyle and expect a new knee to be just like the old one. It won't be.......and there is a lifestyle adjustment. However, walking for long periods of time is perfectly acceptable for a new knee........but playing racquetball, for instance is not.... I think you have to establish a rapport with the doctor and work on them over time. Try everything under the sun, let him/her know you have tried it all......... I'm sure one of our posters, Ann, can tell you more about getting a TKR, and I believe she is in her 60's!!!! It behooves you to become a REALLY involved and educated knee " geek " . Understand what's happening and the chances are that you'll have a good rapport with your doc. Well I had arthroscopic surgery in April for the right and July for the left. They diagnosed with Grade IV chondromalacia, they took out part of the meniscus on both he said they were shredded, and took out the oseophytes ( he said it meant bone spurs) no cartilage on half on each knee. It helped a little but not totally. Then on the right knee he gave me a series of weekly injections of Synvisc. But he said that it needed cartilage to adhere to and didn't hold out much hope for a good result. The left was no cartilage at all so he refused to give it for that one. Any way he says since I am so young he wants to wait till I'm 60 to replace them! Well I am beside myself now and I was grasping at anything the other day when I saw him. The last hope for me is these things that are called uploading braces. Has anyone ever had these? They are a bit pricey and with a tight budget I hate to buy them if they don't work. He said the right I might be able to have a resurfacing ( I guess it's like half a knee replacement) but the left I needed a total knee replacement. I guess they only last 10-15 years and that's why they don't like to give them to people under 60. But I don't want to be like this for 13 years! Thanks for any help you can give me. After 6 years of no life, I decided to get a TKR. I joined 's Joint Replacement group and there are a lot of people there your age or younger who've had TKRs. I've learned a lot about TKR's there. My surgery is scheduled for Oct. 19. You may want to look at the DePuy rotating platform knee (allows the most ROM & is for active people) -- there are 2 levels of mobility, the RP, http://www.jointreplacement.com/xq/ASP.default/mn.local/pg.content/joint_id.6/co\ ntent_id.388/joint_nm.Knee/local_id.45/qx/default.htm, and the RP-F, http://www.jointreplacement.com/xq/ASP.default/pg.list/list_id.113/mn.local/join\ t_id.6/joint_nm.Knee/local_id.36/qx/default.htm. The RP-F allows even more ROM and rotation than the RP but is more expensive and some hospitals won't do them. My doc told me that I would be able to bicycle, hike, and climb indoor rock walls once I got my new RP (either one). I want to be able to use this machine: www.rowbike.com so I'm fighting for the RP-F. The other source is Zimmer, http://www.zimmer.com/z/ctl/op/global/action/7/template/MP/id/9355/prcat/M3, who make a female-specific knee (women's knees are smaller and the Q-angle (you may have to look that up) is greater than men's). From the page above, I clicked on Overview (under Gender-specific) but there's something wrong with their server right now (I called them & they're working on it) so I couldn't get to the info. My OS says that the gender-specific knee hasn't been in use long enough to know whether it's better than the dePuy RP or not, so I'm going with the dePuy RP knee. I had to see 8 OS's before I found one who (a) used the dePuy knee (I now know that the dePuy site has a physician locator link -- and so does the Zimmer site), and ( was willing to do a TKR since I still have good articular cartilage where I don't have chondromalacia divots. Ya just have to keep trying. You may have to use tears (I did). You'll probably have to convince them that you were very athletic and that this has ruined your life, you're depressed, you've thought about suicide, etc. And that you'll devote your life to the rehab. Re the rehab, you have a better chance of ending up with full function if your legs are strong before the surgery. Do everything you can stand to do to strengthen them for the last 6 weeks before the surgery, even if it means skipping other stuff that irritates your knees. I do half an hour on the treadmill at 8 degrees and 3.2 mph, then 15 min on the exercycle, which hurts my knees more. Have you tried knee tape? That might help you do the exercises. If you get a TKR, how long it lasts depends on what you make it do. Many of the newer knees are lasting 18-20 years now, and by the time you need a new one, they'll probably last longer. You can usually only have 1 (maybe 2) revisions (i.e. new knees that replace the original implant). The less your femur and tibia are worn away at the first implant, the less they have to cut away, so the more likely you are to be able to get a 2nd revision. I used the argument that my lack of exercise had weakened my heart (which I found out by falling off a ladder -- I felt like I was going to have a heart attack, went to emergency, they kept me overnight for observation), that heart attacks & strokes run in my family (which is true, but even if they don't run in your family, it's worth the lie), and that not getting a TKR was going to shorten my life. It worked. There's also a new metal (I think it's a metal ceramic, although not being an engineer, I don't know how metals can be ceramics) that's being used for implants called Zirconium (look up zirconium knee). However, the advantage of the RP knee is that it better mimics how a real knee bends. It allows the lower leg to rotate (yes, the lower leg rotates when you bend you knee). Also, the RP implant isn't built on the premise that a real knee acts like a hinge. Real knees do not act like hinges -- the femur slides forward on the tibia (on the menisci). Good luck. Yesterday I got Rahselle's CD, of just the breathing, and put it in my car, so as I was driving around Sydney, Australia, I was doing some deep breathing! I felt really fabulous. I've also shared LifeLift with some of my friends and I know of at least one person who has purchased the LifeLift DVDs.....it will be life changing for them as it has been for me. He told me he thought I had saved his life! In a few weeks I will be driving up the coast to spend some time at The Great Barrier Reef. I bought Rashelle's breath CD so I could breath along with it while I drive through parts of Australia I've never seen.. Oh Pat, I love your tapping and breathing idea! My name is Lindy, I am 34 . Married with 3 small young children. I am brand new to the group and Brand New to Life Lift ( I should be receiving my order in a few days). I am excited about using this system to make me feel better. I have been feeling VERY tired lately. I saw a Natureopathic ( sp?) doctor about it and after lots of tests she concluded that I have severe Adrenal Fatigue. I began my journey to wellness in earnest about 4 years ago. At that time, I was 43, overweight, had undiagnosed hypothyroidism and anemia, and generally felt awful. It took some time and lots of little changes, but I'm now soon to be 47 (Sept. 19th), have recently lost 30 lbs, have my blood pressure, cholesterol, and blood sugar under control. I stay very busy homeschooling my kids (dd 19 - homeschooling college now, ds 12, and ds 8). I'm also taking karate with them and am about to test for my second degree blue belt. After that, I'll officially be halfway to black belt. I'm telling you all this to let you know that there is hope . . . Thanks to the changes I've made with my doctor and from doing LL regularly, I'm feeling better than I felt in my 20s and 30s. I'm now wearing my dd's size 8 and some of her size 6 capris. I still have 20 lbs to go to reach goal, but I'm smaller at this weight now than I was years ago at the same weight (I wore 12s and 14s back then). I had a really big thrill recently - - you see, years ago, during my thin days, I did runway modeling. I always loved putting on the pretty clothes. What a way to work! Just 2 weeks ago, dd and I modeled in a bridal fashion show - she was a bride, I was the mother of the bride. You can't imagine what it felt like to do this again - - I hadn't modeled in 25 years! I can't say enough good about the LL program. I NEVER dread doing it like I did other exercises. I've gained so much strength in my arms. I'm also able to do leg lift stomach exercises at our karate class with ease while the teens in the class moan and groan. I recently started being very faithful about doing the facelift portion of the workout. One of my recently developing expression lines on my forehead (where folks are getting Botox) is beginning to vanish. Here's another idea for you - - I'm taking a product called BioAdreno to help with the adrenal fatigue I experienced. Once I started taking it, I started sleeping like a baby. It's made by a company called Biogenesis. You'll have to google to find out where you can order it. It was recommended to me by my doctor. Hi - I have had Chondro for over 25 years now and if I had to do it again, I would have no surgery at all - not even a scope. That's my opinion only. I've had three procedures on my left knee for exactly that pain/gravel sensation/soreness right under the kneecap. None of them did any good and I'm not so sure they didn't make the problem worse. I would, if I were you, find things that you can do to stay healthy, and give up those things that aggravate your problem. In my case, I gave up running about twenty five years ago, and switched to cycling. I then got a good 20 years out of intense cycling before it caught up with me on the bike. For the last five years, I have done light walking and straight leg raises only - even though the rest of my body is ready to compete on a high level again, the chondro keeps me sidelined. I am in a wait and see mode - maybe science will find a cure but I don't think there is anything out there right now. I have tried everything including four surgeries, and the Jobst compression thigh high socks seem to do about as much good as everything else combined. It is a cheap and safe trial.
