Question regarding ARV to reduce Viral Load in Breast Milk

Dear FORUM,

Can any body give me the references for ability of Anti- Retroviral therapy (ART) in mothers to reduce the Viral Load in Breast Milk .The questions to be addressed are

1.Whether ART can reduce the Load from the day child is born

2.Optimum pre delivery duration of ART required for reducing the viral load .

3. Side effects of drugs in neonates and later on residual effects during infancy .

4. The combination of Ceaserian and ART to reduce the viral load. Prof. A T. Kannan,
