I do need to go to physical therapy to learn how to use it. but I will wear them with SAS shoes,. simply for the padding. The shoes are made of leather top, anti skid rubber bottom. The bottom of the shoes itself is filled with shock absorbent material. The shoe is hand sown. You can get them in all kinds of odd sizes no matter how wide you foot is they got the size for you. You can remove the insert and wash it. It is made to be removable. This insert is soft and has the memory that remembers your foot print. I was wearing when I hurt myself clipping my toe nail to ease the pain. I guess the soft real leather top and the soft and rigid memory of the insert added to that the light weight made all worth the $100 I paid for them. One advice, do not buy the dressy shoe. The sport shoe looks more bulky but has more shock absorption and more grooves to prevent skidding on smooth moist surface. There is a specialty store where I live for work shoes (steel toe shoe for construction) that sells SAS shoes. I can not find the company on the web, but there are a lot of articles about the company on the web. They seem be a good company. A good honest hard working American business. I bought Rockport's, did not like them they are stiff. I used to buy new balance but about two years ago they changed the material of the insert and started giving me problems.
