I am sending this on to all since I have been asked what problems I have to

get SSD the first time I applied. Think hard about these when you fill your

paper work out or when they call. And please remember to make a copy of

everything you send to them. You well be surprised how many times you well

refer

back. They do ask the same question 5 different ways so having copies to

reference well help a lot.

You may think of other things but here's a list of mine.

When filling out forms for SSD think about these suggestions to ponder.

Can you zip zippers, button buttons, ties shoe laces of all kinds or hook

hooks?

Can you cook big meals or do your hands and legs give out before it's done?

Can you walk many steps before you tire out and just how many can you walk?

Can your open a can with a can opener that you turn the knob to do it?

Do you ever lose your balance, sway from side to side, fall forward or

backwards?

Stand without losing your balance?

Walk on uneven ground with tripping or falling?

Walk ok at night?

Walk on ice, slush or snow without slipping or falling?

Open up a new box, twist off caps or open jars the first time or do you have

to wait for someone to do it for you?

Can you walk up inclines without your legs tiring out?

Can you walk down inclines with your legs keeping up or do you seem that your

are going faster than your legs can go and you lose your balance?

Can you sew a button on or hold a sewing needle on?

Can you use heavy pots or pans or skinny handle cooking utensils?

Do you need that shopping cart for stability when you go shopping?

Do you need a wall or something to lean on to keep your balance while

standing still?

How many steps can you walk before your legs tire out?

Can you use a skinny pen to write with?

Can you open a cereal box or any other box without problems?

Can you unscrew bottle caps or get that little piece of foil off under that

cap without any problem or is not enough strength to do it?

Can you cut thick meat easily?

Can you handle that baking sheet ok or is it too awkward?

Do you find yourself buying plastic bottles or jars because you seem to drop

'em a lot?

Can't walk much without stopping for at least 5 or 10 minutes rest because

your legs tire out?

Having trouble cutting the grass, your legs tire out, you tire out, your

hands go numb?

Can't trim those bushes, arms and hands go weak?

Can't take a shower and wash your hair, you lose your balance?

Can't take a shower without a shower bench or chair?

Can't take a bath because you cant get back up in the tub to get out?

Have to wait for someone to come over to open that jar of pickles or peanut

butter because you cant twist off caps or lids?

Can't go up those stairs without a railing due to no strength in the legs to

get you up there so you have to pull yourself up?

Can't go down those stairs without a railing or you end up there before your

legs do?

Can't dust or vacuum because arms and legs get tired before you get done?

Can't iron, hand go numb before the wrinkles are gone?

Have to use fat handle cooking utensils?

Yard work, yikes...can't keep your balance on the lawn, or trim the bushes

without losing your balance?

Can't move furniture, not enough strength?

Have a HANDICAP sticker or license plate?

Can't lift heavy objects or medium weight ...too heavy for you over 10 or 15

pounds?

Can't stand still without going forward, backward or side to side?

MAKE SURE all your DR's have all your information and any thing else you

can think of to add to your list. Even the smallest thing can make a

difference. Social Security well contact every Dr.
