Obtaining Your Complete Medical File

Hi all,

I have reason to believe that my previous doctor has incorrect

information about me in his medical file. This concerns me, but what

is more important is what he has reported to the national database.

(Note: I say " national database " for lack of a better term, I know

there is some sort of way that medical information is compiled into a

big database. There may be more than one.)

I would like to obtain a complete listing of all info on me contained

in any and all of these major databases. The problem is that I don't

know where to start. Any ideas? My web searches have so far been

unsuccessful because I don't know the names of any of these

databases.

Any input is greatly appreciated.
