I think answer is smoking.According to master dentistry volume 2 ,page number 22 -Smokers have higher prevalence of pdl disease,smoking increases periodontal destruction by three to six fold. According to page number 13- HIV infection predispose to necrotising ulcerative gingivitis that may progress to necrotising periodontitis. Both smoking and hiv infecction causes periodontitis. According to carranza,most cases are predisposed to smoking in necrotising ulcerative periodontitis and most cases of necrotising ulcerative periodontitis are predisposed to immune deficiency virus ie HIV.hope it clears the doubt.
