They're hiring an American to take care of Americans on an American Government contract, with specifications set by the American Government. Makes perfect sense to me. Since I don't recall much drinking and carousing in downtown Kabul during the evenings, maybe it's just as well to keep one's self occupied. <g> And as for American nurses; while it does seem to be written with an American flavor, I don't know that they would not accept interested candidates from the UK or other places. Certainly this remote medic list is UK-based, but I get the sense that there are a fair number of American members reading these posts.
