The only concrete statement that I ve been able to find is " Lesions with moderate or severe dysplasia may be excised, ablated by laser or treated with topical chemotherapeutic agents such as bleomycin. Occasionally patches are too large to treat in these ways and the only option is to monitor to detect malignant transformation as early as possible. " (source-odell 3rd ed case 45- white patch on tongue) I believe the actual management varies from case to case, after taking into consideration the risk factors that may be involved.
