I have a speciality in Orthodontics.

1. These depend on the appliance you use. For example, when you use headgear, you can adjust inner facebow roughly using the cast before patient comes in. When you fit bands, you can choose the band size roughtly looking the size of teeth on the cast. Etc...

2. It is a very vague question. There are lists of examinations you need to do for the diagnosis of malocclusion.

- interview - complaint, age, sex, habit, tmj problems,

pshycological status, previous ortho

- extra oral exam - skeletal and soft tissue exam in transverse,

vertical, anterior-posterior planes,

tmj problems

- intra oral exam - dental health - caries, restoration,

periodontal status

- individual arch - tooth number, shape, size,

location, rotation,

crowding/spacing, inclination,

canine angulation, curve of spee

- occlusion - incisor relationship, overjet,

overbite, canine and molar

relationship, crossbite, centrelines

- Records - study models with bite wax, photographs(intra/extra oral),

Radiographs

After analysing above records, treatment plan is made.

There are very basic, so you can find it in any orthodontic textbook. Did I answer to your questions? If it's not clear plz ask me.
