A) I think in this case you inform the patient that there are other treatment options which might be more suitable in his case. he's overweight, smoker, hypertension. Things to be cover: 1. Why does the patient want the extraction under GA? 2. Does he know about the side effects of GA (should not eat anything for 6 hours before operation, nausea and vomiting after the operation, should go home accompanied, should not sign any legally binding documents for 24 hours.for him risk of chest infection, slow recovery. 3. This will most likely be a simple extraction since there was little bone around the tooth. 4. Does he know of alternative forms of anaesthesia? Actor will say no. Then talk about LA, oral and IV sedation. Describe each with advantages and disadvantages (don't say cannula as actor will pretend he does not understand it). You can get the advantages and disadvantages of these from Kathleem Fan. 5. Ask if they have any questions and tell them that the final decision is theirs. Q) Photograph of perio instruments (Sickle scaler and Gracey's and ultrasonic scaler). Questions about their mode of action, what are the advantages of the ultrasonic scaler. A) Sickle scaler has a supragingival while Gracey is subgingival. You can get the advantages and disadvantages from Kathleem Fan. = Scalers – pictures to identify (1) sickle, supragingival, (2) curettes, subgignival, (3) ultrasonic, cavitron.
