help needed : written treatment plan format ore 2

Can some one please clear my doubts with written treatment plan format.

1) Do we need to write emergency treatment as one of the option or not.

If yes, plz help me with the case of TOOTH SURFACE LOSS

IN EMEGENCY, WE GIVE DIET ADVISE, WHAT IS THE ADVANTAGE AND DISADVANTAGE OF ANY

ADVISE WE GIVE.

2)What all needs to be added on the blank page which is on the back side of the

written treatment plan. Should we write short description of treatment, oral

hygine instruction, dietary advise, maintenance phase etc.

3)should we include referral as a treatment option , If yes what abt advantage

and disadvantae, If no, where we should write refeeral on the blank page?
