Guest guest
I think the answer is correct as the BNF confirms that on page 721 of the 62nd edition(sept 2011) What is the best treatment for a patient with cured denture stomatitis but resisted angular cheilitis
Guest guest
Hey shipra,Firstly in denture stomatitis, the cause shud be identified. For example, if the flanges of the denture are overextended,,simple correction of the denture solves the problem and no steroids are required.Conversely, if the problem is refractory,as in case of type 2 diabetes,,HIV,,or similar systemic condition which predisposes to opportunistic infections like candida infection,,a faulty denture may cause a similar condition and is now called candidal denture stomatitis...and HERE if steroids are used they will increase the problem and hence contra indicated.Hope it clears ,,,-Ashish.Sent on my BlackBerry® from VodafoneFrom: Shipra Bapna <bapnashipra@...>Sender: Date: Sat, 24 Mar 2012 07:39:04 +0000 (GMT) < >Reply Subject: Re: Another ORE Question Hi ashish. What do u think abt hydrocortisone for denture stomatitis won't it be C/I From:Shipra Bapna <bapnashipra@...>; < >; Subject:Re: Another ORE Question Sent:Sat, Mar 24, 2012 5:58:17 AM Hi Dr..hydrocortisone is a steroids and is a predisposing factor for oral candidiasis...plz can anybody clear my doubt ...RegardsShipra.From: Ruchika ; ; Subject: Re: Another ORE Question Sent: Fri, Mar 23, 2012 12:46:17 PM ThanksFrom: Gmail " " Sent: Friday, 23 March 2012, 12:31Subject: Re: Another ORE Question Miconazole with 1%hydrocortisoneDr S.S. Guest guest
hi allthanks for all ur answers5.3 Angular CheilitisAngular cheilitis in denture-wearing patients is usually caused by infection with Candida spp. andthere is an associated denture stomatitis that should be treated concurrently. In those withoutdentures, angular cheilitis is more likely to be caused by infection with Streptococcus spp. orStaphylococcus spp.Miconazole cream is effective against both Candida and Gram-positive cocci and is thereforeappropriate to use for all patients. Where the condition is clearly fungal in nature nystatinointment can be used and where it is bacterial in nature sodium fusidate (fusidic acid) ointmentcan be used. Note that creams are normally used on wet surfaces whereas ointments are normallyused on dry surfaces.Unresponsive cases can be treated with hydrocortisone and miconazole cream or ointment.Continue treatment until clinical resolution is achieved. A lack of clinical response might indicatepredisposing factors such as a concurrent haematinic deficiency or diabetes. Refer such cases toa specialist or the patient’s general medical practitioner. If dentures themselves are identified as contributing to the problem, ensure the dentures areadjusted or new dentures are made to avoid the problem recurringTHIS SECTION IS FROM THE SCOTTISH GUIDELINES 2008SO I BELIEVE WE WILL STICK TO THE GUIDELINES AND THE BNF2.
