Basically the place where you have your DCI registration done, in whichever state. They take a week and charge small fee. You have to provide a covering letter stating why you need the certificate. It is very easy. Just contact the local state dental council office. I got one letter from my country but they need one from the country of graduation which i have only did my internship.
