Even though it is very pertinent to draw the attention of NACO time and again, we need to realize that some things are beyond the official rules and systems. Inspite of having a very systematic method of intervention, if we cannot change the outlook and the attitude of the doctors and service providers towards the PLHA, then how effective that systematic management will be? No doubt that we need a proper system for an effective response but when we look at the ground realities, the perceptions and the attitude of doctors need to be in tandem with the system otherwise the desired outcome from the systematic management may not be achieved. In the context of Manipur, as someone who has been working in this field for more than a decade, we have come across cases of doctors prescribing Nevirapine three times a day which is something not called for. There have been cases of differing opinions among doctors of Manipur in prescribing medicines for O.I.s along with ART. So what we feel is that there should be a continous medical education for doctors from within their community by experienced professionals to tackle the issue in a more effective and humane approach.
