Authored by H. I. Blanks, Ph.D., Tonya L. Schuster, Ph.D. Marnie Dobson, B.A. It measured health using a unique health survey instrument developed to evaluate health as defined as, ". . . a state of complete physical, mental, and social well-being and not merely the absence of disease and infirmity."
