Guest guest Posted May 23, 2002 Report Share Posted May 23, 2002 Holly - I am going to type in the body of our peds letter of medical necessity in case you want to do as recommended about having a letter ready for the dr to copy- which is a great idea! Here it is: I am writing this letter on behalf of my patient Dane Manias. Dane has been diagnosed with torticollis and non-synostotic positional deformational plagiocephaly. Dane has been evaluated and will receive physical therapy for this condition. Due to the severity of Dane's plagiocephaly, treatment with a DOC Band (procedure code E1399NU, Diagnosis code 754.1) is indicated as the most effective intervention at this time. If his condition is not treated adequately, he can have long-term temporal mandibular joint problems, significant facial asymmetry, and possibly hemi- hypoplasia. The cost of this therapy to his family is substantial if not covered by insurance, and Dane should receive the most effective therapy as soon as possible to give him the best chance to avoid any complications from this condition. I am asking that his DOC Band and therapy be covered under his insurance as a medical necessity. Please do not hesitate to contact me if you have any questions. Thank you very much.
