" so....since 85% of virtually all medical care is 'non-compliant'.... perhaps that says more about the nature of the bureaucratic notion of what chart notes are supposed to be, rather than notes for the provider (and various legal agents)....and in solo practitioner situations, just who else is responsible anyway? If they want a signed report, perhaps they should request such. If they want chart notes....has anyone looked lately at what is considered 'acceptable' justification for any kind of care? At what point does ot become feasible to hire professional writers to generate whatever the current level of compliance requires...
