Referral suggestions for patient with rotator cuff tear?

I have a patient that is in need of a shoulder specialist and am

looking for suggestions/possible referrals?

The MRI report states ... " 8mm x 6mm (AP x TRV) intrasubstance

tear/tendinosis of the anterior and insertional fibers of the

infraspinatus tendon. No evidence of articular or bursal-sided tendon

injury or discontinuity "

PT, massage and CMT have not helped.

Your recommendations are appreciated.
