Charge workers comp fee   $10. First page and .50 per page

thereafter. Count the total number of

pages , calculate the fee, send a letter to the requester that  “

upon receipt of $xxxx  the copies

will be sent.”

BE SURE they have a State approved records release signed by the

patient.

I received a request for chart notes for an MVA from the non-pip insurance;

does anybody charge a nominal fee for this? And if yes, how much? And how do

you bill for it?
