Guest guest
Mitzi has recently returned to the OCD-L and posted a request for individuals to fillout her questionnaire with the answers to be features in a book on " Helping " childern and teens with OCD that she is wriiting The questionnaire which follows appears to be one of information gathering.: a survey. As it is now constructed, it appears to be just the accumulation of many questions I have seen on other questionnaires. Personally, i see it as a new resource to document OCd in our family [including myself], and plan to use it to that end at the least. Re: OCD Questionnaire for parents >>There are nearly 80 families subscribed to the email list. >>Would you like me to post your request there ? Or perhaps the questionnaire >>itself. > >Thanks -- I would very much like to post my questionnaire to your list. I appreciate your help :-) Please remind folks to send their completed questionnaires directly to me rather than to the list, though (unless they would also like to discuss them there, which is fine as long as they also send them to me) -- Mitzi Waltz > > >============================== > >==================== > >OCD Questionnaire July 3, 1999 > >Dear patients and parents: >Thank you for offering to help me with my research. My book has a working title of _Obsessive-Compulsive Disorder: A Guide to Helping Children and Teens_. It will be published in Spring 2000 by O¹Reilly & Associates as part of its Patient-Centered Guides series. This is my third book for this series. For more information about O¹Reilly¹s Patient-Centered Guides, see http://www.patientcenters.com/ >I¹m sending these questions to many teenagers with OCD who have expressed interest, and to the parents of younger children with OCD. I will quote from your answers in my book. I truly appreciate your insight, and I know that reading about your experiences will make this a better book for others who need information and help. This is by no means a scientific survey, but our experiences and opinions count a _lot_. We are the people who have experienced these disorders in ourselves or our children first-hand. >Please feel free to make your answers as long as you wish. If there are questions you don¹t want to answer, that¹s OK. If there are questions I haven¹t asked that you think should be addressed, tell me more. You can copy and distribute this questionnaire to other people you know who might have something valuable to say. -- Mitzi Waltz > >Please return your questionnaire via e-mail to: infobahn@... >or via mail to: Mitzi Waltz, 75 NE Going St., Portland OR 97211 > >_Questions for Teens With OCD/Parents of Children With OCD_ > >First name: >If you are a parent, child¹s first name: >May I use the information above in the book?
