Jessie >> In my daughters case, a Functional Behavioral Assessment included: BASC questionnaire to parents, teachers, student, administrators, with subsequent scoring and evaluation Classroom and non-classroom observation Individual interviews with student, teachers, parents Motivational Assessment Study A written report This was done by the school's team of special education teachers, school psychologist, etc. Now I have had non-school driven behavior assessments done when she was much younger, including one done at a psychiatric hospital where she was on day treatment in first grade, but I don't remember all the elements.
