Hi Kai,

Thanks for your post. Responses are going well! The Ladies are

still in the lead though....how 'bout it guys! Remember, Caregiver

stories are welcomed, too! And, a thank you to Tracey for all her

help for allowing the post and placing it in the files section.

K, I will email you a copy as well. But for anyone reading, here's

the information and the questions are shown below.

There is a file, in the files section and I've included the contents

below. It's a word template that will allow you to type your

responses and then save for emailing. If you have problems with the

file (a couple of folks have had a software compatibility issue) just

email the responses to me at gstephens@....

Thank you to EVERYONE who has responded! Keep them coming! I think

you are going to be quite please with how it is turning out. I
