update on vent pt s/p gastric bypass '03

Update on pt below:

has 50 cm of jejunum left.

Output from gtube >1000 mL

NGT ~500-600 mL

She is currently receiving ~2500 kcal but not by my choice.

She just started on vit b12.

Pt s/p ex lap small bowel resction & s/p spontaneous delivery of still

born. Small bowel was gangrene but remnant stomach & colon healthy.

NGT inserted into roux limb & gastrostomy in body of stomach for

decompression. NGT output yesterday 540 mL. No diet PTA.

Current wt: 209#, pre preg 182#, ht: 5'6 "

Currently on ppn.

Please provide advice on how to calculate needs and recommended diet.

I think about 2000 kcal/d is adequate with 135-150 gm protein/day.

When possible, start low rate tube feeds with an elemental formula.

That would be my best guess.
