Hello, My name is Charlotte . I am an outpatient RD at McKay-Dee Hospital in Ogden, Utah. Recently, I have received several referrals from a MD who performs gastric bypass surgery. My knowledge on this subject is limited. This MD has a dietitian that works with pts after surgery. He wants me to evaluate the pt prior to surgery. (He probably needs someone not connected with him and for insurance approval.) Anyway, Does anyone have an assessment form that you would share with me or any information would be helpful?
