Attached is my current Excel form with some generic data I made up. I've been using this form to give a daily 1 to 10 evaluation of things I think are important. Mostly they are a general topic like "headache". The form as two columns per topic. One for the number, two for any notes for the day. The bottom of the form is set up to give average, average deviation, max, min, total and count. The week number and dates calculate from the two blue text boxes for start week, and start date in the upper left. After the first couple of weeks I found many symptoms don't move much so as I start a new week I copy the last number down (paying attention to formatting). I'm finding this form useful.
