The Alliance group is working together to:

Develop new and innovative services to achieve the best possible outcome for people living in the community. Improve the quality, efficiency and effectiveness of our service through research and the sharing of knowledge. Strengthen the management of chronic disease in the community to reduce hospital episodes for people with chronic conditions. Work with GPs and other health providers to improve access to primary health care. Build on a well co-ordinated, technologically advanced community infrastructure to take the pressure off the acute care hospitals. Work with others to test and develop innovative service models that keep people independent and at home. Ease the transition from hospital to aged care in the community or a residential facility by providing a better interface between hospital and aged care Support the training of undergraduate and postgraduate health care workers through clinical placements in community and residential care. Assist in attracting nurses and other health professionals back into the workforce and in particular to community and residential care. Make better, 'smarter' use of medical, nursing and allied health skills through use of technology and service models that spread these skills further than traditional models of service delivery. Work with rural communities to ensure that they have access to health services through innovative models of care and better use of technology.

The synergy between the Alliance key goals and the objectives of this international conference on community health are apparent, and with their key support at Platinum Sponsorship level, this conference promises to be an outstanding event not to be missed. Telehealth

In this edition of E-News, we highlight another of the conference themes: " Information Technology and Communication Systems " . The delivery of health-related services and information via telecommunication technologies is known as telehealth, an expansion of telemedicine, encompassing preventive, promotive and curative aspects to provide a myriad of technology solutions. An example of innovative telehealth is the recent trial undertaken by the Royal District Nursing Service of SA (RDNS). RDNS is a non-government, not for profit organisation which provides mobile nursing services and other home and community care in Adelaide, South Australia. This trial was a pilot project to assess the suitability, safety, and cost-effectiveness of delivering community nursing services from RDNS via a videophone link from the RDNS call centre (RDNS Direct) directly to clients' homes. Clients were enrolled in the trial to receive daily medication management via Videophone. They were from two groups: elderly clients with cognitive impairment, and clients receiving Directly Observed Therapy (DOT) for tuberculosis. The results demonstrated significant benefit to the clients, RDNS and the wider health system. Due to the successes of the trial, the service has now been adopted as part of the RDNS service delivery model. Outcomes of this innovation will be presented at the Health in Transition: Researching for the Future conference in August 2009. If you are undertaking similar projects in the Information and Communication Systems field, we invite you to submit an Abstract now

Australia has been fortunate to be invited to host this prestigious event - the 4th international conference on community health nursing research. This international conference is held every four years and previous locations have included Canada, the United Kingdom and Japan. The Host Committee extends a warm welcome to visitors from around the world to Adelaide, South Australia August 16-19th, 2009.

Sincerely,

The Host Committee Health in Transition Conference 2009
