Guest guest Posted June 23, 2004 Report Share Posted June 23, 2004 My daughter will probably be emailing you because she is the one that had her explant with Dr. Lee on 4/16/04, but I was with her with every meeting with him and can tell you we were both extremely pleased. He did an en bloc through the crease. We told him this is want he wanted and he told us he had done many, especially with silicone implants. We also old him we wanted the implants to send to Dr Blais in Canada, and he was fine with that too. We found him to be very sympathetic and amenable to everything we wanted. He also didn't try to talk her out of the surgery or make comments to the effect that she would be unhappy with the look of her breasts afterwards. We did ask about a lift, but because my daughter is 25 and has never had children and had only had them in less than 2 years, he didn't think it was necessary. She has pretty much returned to her former size. Health wise she is so much improved and feels like she has been given a second chance at life. Hope this helps and good luck. I do not have alot of natural breast tissue either, but I definitely sagged from my breast feeding and my doctor did recommend a lift at the time of explant. I was glad I did, because I did not want to go back to my awful sagging. The en bloc removal ensured that all of my scar tissue came out. I do have the anchor lift scars. I believe there would have been a possibility that I could have had the vertical lift that is a newer technique and would have looked much better. If you have saline implants, en bloc may not be necessary, but a total capsulectomy IS. You could have the implants removed through the nipple, as long as you get it in writing that every effort will be made to remove all of the scar tissue. Sometimes doctors will claim that they can't get to it all, and will leave the posterior portion of the capsule...that is where you will have to determine if the doctor is going to have your health in mind, rather than his surgical schedule. Getting all of the scar tissue is time consuming, and if they don't beleive that your health is at stake, some doctors will be careless about getting all of it out. They just don't think it is a big deal. But when it comes to health, it can be a VERY BIG DEAL! So, bottom line is that you don't have to have en bloc if your implants are saline (if they are silicone gel, then YES, you do need en bloc). You do need a total capsulectomy for sure if they are saline. I've heard from Ilena that Dr. Lee is an excellent plastic surgeon, and I do believe that he will do a great job for you. I don't think it is necessary for you to travel anywhere else if he is convenient for you. It sounds like he will do what you ask, which is a great thing, and I think he will make you look great too. Hope this helps you feel better about it--I know the en bloc and total capsulectomy issue can be confusing, but the focus either way is on getting the scar tissue out along with the implants if you are sick. The benefit of the en bloc is that it protects you from contamination if there is a spill inside your chest. I did create a Frequently Asked Questions section in our database and answered that question there. Here is what I wrote, with a little further embellishment: What is En Bloc? En bloc refers to the method of removing an implant along with the scar tissue that surrounds the implant in one unit. It is primarily used for removal of silicone gel implants to ensure that if there is any loose silicone gel from ruptured or leaking implants, that the gel does not escape into the chest cavity and contaminate surrounding tissue. Think chocolate covered cherries....if you break open the chocolate outer shell, the liquid center spills out. Removing en bloc means removing the implant without breaking open the scar tissue shell, so that nothing escapes. When an en bloc removal is performed, a total capsulectomy will be done as well, since the entire scar capsule will surround the implant, keeping the contents contained. What is a Total Capsulectomy? A total capsulectomy refers to the complete excision of all scar tissue that has formed around an implant. It will be removed at the time of an en bloc removal due to the nature of that procedure, or it can be removed separately once the implant has been taken out of the body. The object of a total capsulectomy is to remove all traces of scar tissue. Scar tissue will always be formed around a foreign object in the body. It is imperative that a total capsulectomy be done at the time of explant, as opposed to a partial capsulectomy. No scar tissue should be allowed to remain in the body, as it can continue to elicit an immune response, preventing full healing. Scar capsules can contain silicone particles, breakdown products, bacterial colonies and other pathogenic material that can prevent total healing. For this reason a total capsulectomy MUST always be performed when implants are removed from the body, for complete healing to be attained and for future protection. If an en bloc removal is performed, this will ensure a total capsulectomy is done as well. Or is a capsulectomy sufficient enough? The plastic surgeon can drain the saline water from the implant (of course, being very careful not to spill any into your chest--you don't want bacteria-infected water going anywhere inside you.). He can then remove the implant shell, along with the scar tissue, thus performing the total capsulectomy. Get it in writing that he will do this, not leaving ANYTHING behind. Go to Shaw at UCLA too since he's close by. The thought of a scar in your crease to by Lee feels *iffy* with me. If you're in the need of a lift (most explantees are), you want a surgeon who feels this is just as important as you feel it is. The enbloc thing is important too.I'd go to a surgeon who could do enbloc, and a vertical lift. You don't want scar creases if you can help it. Take it from me--I have them and it makes me sad(though mine are bad). I so envy those who went to good surgeons like Feng, and Spear to be explanted. got a good explant without a crease scar--I think. Her pictures look very good. I saw them yesterday. I had enbloc removal and I had a lift with very little breast tissue and it all worked out just fine. This is not true that you have to go through the crease and I have to tell you from some of the surgical procedures I have seen done through the crease I know for sure that this is not a good way to do the lift on many patients, these scars just don't look good on many of us. I am sad to hear that Dr Lee does not do this the way it can be done and the way it should be done. I know that Dr Feng is pricey but she does the most wonderful job at getting EVERYTHING OUT and some implants such as mine are tough to get out. I had those nasty textured things and they were a total nightmare and yet I had them removed enbloc through the nipple incision with a nice vertical lift that left me virtually scarless and NO CREASE INCISION! I think some Dr's are not capable of this type of commplicated procedure or, they really don't believe that implants cause this so they really don't believe it is even necesary, I don't know, I don't understand it at all, but I know that it is BS that there has to be a crease scar. Please don't go to any Dr that tells you there has to be a crease scar, it is a lie. But I probably would not want this Dr to do my surgery anyhow because he probably would not do a good job if he did not use his own method. Every Dr is different and I just think if it was me I would go to the ones that def know how to do en bloc removal without needing to give you scars in the crease area. I recently saw some women with crease scars that looked so bad I felt like crying and thought WHY ON EARTH WAS THAT NECESARY. It is not necesary that is why. Please please please don't let them do that to you. Find another Dr. If you cannot afford to go to Ohio then at least check out Mehlmed I know he does not always like to do lifts on his patients at first and that also can suck but at least I know he can do en blocs without having to use a big huge crease incision. Amnyhow I am not saying all crease incisions look bad, I have seen others that look ok, esp on women with enough tissue to cover that area, but my beef with it is that if there is a way to do it with less scarring why not use that method ya know? Guest guest Posted June 24, 2004 Report Share Posted June 24, 2004 Sorry But I disagree with Patty on this one, I still think ENBLOC is the best way with saline or silicone because the saline can be full of gunk that can spill and be dangerous and I just don't feel that any Dr that won't do enbloc through the nipple and vertical mastopexy is not doing the best he can. Guest guest Posted June 24, 2004 Report Share Posted June 24, 2004 That is not always the case, I had enbloc with no crease scars and so did many many other patients of many other dr's I know of, not just Feng. It is unnecesary to have those scars, Many of us do need a lift and the lift will make our breasts sometimes sit up quite high and the crease scars are often visible because we have no tissue to cover it. My opinion is get some other opinions. I would not want a crease scar as I have seen them and for some reason they just don't seem to hide or heal as nicely as the ones around the nipple or the vertical scars. I asked Dr Feng point blank if those crease scars are ever needed and she told me point blank they are totally unnecesary. However I know a few patients that she did explant through the crease with tiny small incisions less than an inch long, when they had no other incisions on the breast at all, and did not want any around the aereola so that later they could breast feed etc, these were usually patients that did not get a lift. Also Jade had recent pics of her explant done en bloc with total capsulectomy with Dr melmed and he did not use any crease scars. I feel if this is important to you then please please seek other Dr's opinions. There is never only one way and when any Dr starts to go down that road it is time to seek another opinion. Even if you have just small breast like mine they can be saggy, I think if you want the lift you have the right to have it done the very best way possible, Either a vertical or beneli or even this new internal lift. From what I gather about the internal lift not everyone is a good candidate for it, but hey it is worth checking out, anything is worth checking out, who wants to be saggy after all this surgery and money? Personally I would rather be flat than saggy anyhow. I think that sagginess really is aesthetically unaceptable when it does not have to be that way, I had almost no breast tissue and my breasts benefitted allot from the lift not just for the lift but to remove the excess skin and to reshape the skin that was lift to give the appearance of more breast tissue than there is. It is a good way to make the breast appear perkier and more youthful and the vertical and nipple scars healed really quickly on me. I know that a crease scar would not have been good on me with me being so perky and so small, I cannot hold a pencil under my breasts at all so any thing in the crease would have been very obvious. Why settle when you don't have to? I mean is Dr Lee that much cheaper that it is even worth it to go through all that surgery and not get what you want and not even know if he really got all the posterior capsule out? One thing though--Benelli lift (with the "purse string") is controversial so says Dr. Louis Morales at Univ. Utah and Armiger at s Hopkins. The purse string used for suturing can 1.fall apart causing widening of the scars 2. infection Concentric(donut) matopexies can be used with better suturing methods than Benelli. Anyway, there are plenty of modern mastopexies that DO NOT require that big in the crease incision. Personally, I'd shop around if it were my situation. I have a friend, she does not have implants but wanted to get them because she has slight ptosis in her A size breasts from yo-yo dieting. I told her forget the implants and now she's getting a concentric mastopexy--a mini breast lift. With her A size breasts, she's a good candidate for this type of lift. My PS uses this technique on small breasts and vertical for medium and large breasts. I had silicone implants for 20 years and during the last two months before the explant they ruptured and they made me real sick. So sick that I was desperate to get them out because I could feel getting worse by the day. Because every surgeon would take at least a month for consultation and another month to two months before surgery, I thought I would become crippled waiting that long. The only PS's recommended here in San Diego were Dr. Lee and Dr. Dobke. I went to Dr. Lee and was scheduled for surgery. As I was getting worse and worse I kept talking to other PS's outside of the state, even considered going to Dr. Kolb, who could do my surgery sooner, and I was even going to go alone because I did not have anyone to go with me. I was so sick that I felt I had to go but thankfully a cancellation happened with Dr. Lee and was up for surgery. I believe he did a great job with my surgery. He removed the entire capsule and did the enblock procedure. He took pictures. I do think the did an excellent job removing them. I highly recommend him. If you want the capsule removed, insist on it and if the doesn't want to do it, get a second opinion. I too wanted enbloc but did not get one on the left side at first because: the surgeon thought they were saline. You see that do not feel they are necessary for saline. It turned out I have silicone and he did remove all capsule. However, there was a silicone spill. You see I told him to do one thing but he did what he wanted. You must be imfactic. Let him know you will be reading the operation reports. All details of the surgery are put there. Tell him why you feel it is absolutely necessary. what PS would like to do is remove the implant then go back for the capsule. Enblox is more of a tedious procedure. However, we need this because dislodging the implant from the scar tissue is like saying ok silicone I've just shook you free, go wild. Enbloc retains all that poison, capsule, tissue and implant all in one piece. From what you said, I would beleave him but stress the point I've made. In addition, the pathology report will state implant came to us enbloc. NOT in two pieces. I'm glad you're doing the drains, they are also necessary. Also, I have a crease scar, and only a crease scar. Im glad I went that way. It works for some, not for all. I also had a lift with it, with no other scars. Dr Kolb uses the same exact crease scar from implantation, in taking them out, and lifting with my explant. According to her-It depends, on how much tissue you have left, and how your breasts are shaped, and if they are high or lower . A few women have since have had the same lift, and told me they look great. I actually did not go to any other ps for their lolliops' as I did not want to have any scars around my nipples( I had a very real bad feeling about infection, although small in my case around the nipple, but I went with it) Glad I did, I was full of infections, and I know in my heart -I would have had problems. There are many other lifts, besides the lollipop, you have to find a ps that does a lot of different lifts, and does not do "certain ones " as they are unnecessary. Unnecessary for whom? All women are different, and need different lifts. Alot of PS's do the same lift over and over in most cases. A lollipop would have looked terrible on me. I would have also lost more breast tissue. Look at my pictures, I didnt a lollipop, as I was told in order to look good, I look great with a crease lift alone. Why cut in more area's when I didn’t need too? My crease scar is almost gone, and healed very nicely. I couldn’t be happier. Im gonna send more pics soon. I had my explant through the crease (with al lift to, although I needed one) and am doing well and normal again! God Bless! Re: Help Please, have many questions...had consultation My daughter will probably be emailing you because she is the one that had her explant with Dr. Lee on 4/16/04, but I was with her with every meeting with him and can tell you we were both extremely pleased. He did an en bloc through the crease. We told him this is want he wanted and he told us he had done many, especially with silicone implants. We also old him we wanted the implants to send to Dr Blais in Canada, and he was fine with that too. We found him to be very sympathetic and amenable to everything we wanted. He also didn't try to talk her out of the surgery or make comments to the effect that she would be unhappy with the look of her breasts afterwards. We did ask about a lift, but because my daughter is 25 and has never had children and had only had them in less than 2 years, he didn't think it was necessary. She has pretty much returned to her former size. Health wise she is so much improved and feels like she has been given a second chance at life.Hope this helps and good luck.
