Guest guest Posted February 21, 2008 Report Share Posted February 21, 2008 Without seeing what the darkened skin looks like (simply a change in color vs dermatitis)it is hard to say what may be causing the color change, but here are a few ideas... Niacin can cause a change in skin color around the mouth, but the skin is usually inflammed. A deficiency this extreme would likely only be seen in a patient consuming excessive alcohol. Vitamin C can cause a color change (scurvy symptoms) that looks like an old bleed. I would find this unlikely unless the patient's diet was extremely poor in Vitamin C and no supplement was taken (the amount of Vit C needed to avoid this is generally <10 mg/day) Any anti-diarrhea medicine with Bismuth salts, including Maalox and Pepto, can cause a temporary (a day or two) discoloring of the skin and tongue Hypothyroidism and adrenal dysfunction also cause skin color changes, generally a more pale color in light skinned people and a darkening in olive color or darker skin. The skin around the mouth is especially vulnerable. He had a leak following surgery and was intubated for 3 days post op. Should I check for any particular vitamins/minerals? Do you think its niacin? He is a recovering alcoholic but I know he is not drinking now. Do a search from the site (black tongue) and you will find it. I recieved some excellent responses with pictures and explanation of possible cause and treatment. What I found is it could be fungal overgrowth from antibiotic use and/ or lack of saliva production. I never found a specific vit/min deficiency related to this and did not find it to be directly related to the patient's bariatric surgery. It is not something I have seen again in any post op WLS patient since that time several years ago. It was an interesting case. We had a pt who was also complaining of a Black Tongue and he was dx with a Red wine allergy by his pcp. Once he stopped his red wine all symptoms resolved. Just thought I would pass this on.
