I use the NCP at our clinic and have no problems with it. All of the dieititians around me recognize this and insurance companies have had no problems with it either. I would recommend you use it. Good Luck! Anne

I am the only RD here and feel like I’m out of the loop. Is this something that the ADA is still looking to make mandatory? If so, will this be used in an office type setting?

I have not been trained on the NCP but have some working knoweldge would you mind sending out the form you use with bariatric pt s following the new outline?

Here's the PES portion of the template I use for new assessments (I include the patient's history, habits, 24 hour recall, etc beneath the problem section). I also use similar templates for follow up, post op, diet and exercise, etc appts. So far this has been working well for me. Anne

Initial Evaluation

___ is a ___ referred to me by Medical Specialist ___ for a nutrition consultation to determine appropriateness of proposed surgical intervention for weight loss surgery.

PROBLEM:

Morbid obesity related to calorie intake exceeding needs as evidenced by BMI = ___, waist circumference = ___ inches and ___% body fat.

SHORT TERM GOAL:

Positive change in food related behavior and exercise to result in 5% weight loss prior to surgery

LONG TERM GOAL:

___% excess body weight loss after ___ surgery

INTERVENTION:

1. Reviewed healthy food choices, serving sizes and appropriate eating techniques; patient should choose low fat, high protein meals and snacks; ___ demonstrates ___ understanding and shows ___ motivation to comply

2. Exercise for a minimum of 30 minutes 3 days a week

3. Take a complete multivitamin daily

4. Refer to Dr. Josh Roller for surgical consultation and follow up with RD for pre op education / Begin _ month supervised diet and exercise program through WLSC / Begin _ month supervised diet and exercise program through PCP / Follow up with RD to monitor diet changes
