REVISED SETTLEMENT QUESTION

Anyone know about the long term benefit...I think I qualify for:

You are not a current claimant, but are registered as an " other "

or " timely " registrant. We received your registration form on

12/6/94 and your Inamed claim form showing your proof of manufacture

(POM)on 9/13/99. To be a current claimant, you would have had to

registered by 9/16/04 along with filing a disease claim at the same

time and the POM would have had to been in the Claims Office by

12/16/96.

You are eligible to file a claim for the Long-Term Benefit Schedule

program until 12/15/10. If you would like us to send you a packet of

information regarding the LT claim form, please e-mail us or you can

call us at 800-600-0311. We have your address as 639 Whiting St.,

#31, Grass Valley, CA 95945. If this is incorrect, please send us

an updated address in writing.
