pre-op clearance letters/info shared

For those that do pre-op nutrition clearance

visits, do you have a clearance letter that you can share? I provide this

service to insured’s with certain insurance because I am a network provider.

Do you make pre-op lab recommendations, vit/min recommendations or not? I

am not connected to any of the physicians performing the surgery.
