I expect these companies to pay my full fee as long as it is reasonable. I use a $45-$55-$65 fee for 98940-2, which I think is very reasonable. A major ins. company has sent me a contract with a conversion factor of $40. I don't have up to date RVU's for our services, but based on 2008 RVU's, I'm looking at $30 for 98940 and about $50 for 98941. Does that seem correct? Am I supposed to negotiate up from there? The same company pays 60% or 80% out-of-network depending on the plan. Not sure what in-network coverage would be. Who comes up with this stuff and how is it legal?
