How do you feel about having an Advance Directive (AD) and

Durable Power of Attorney for Healthcare (DPAHC) as related

to your CMT?

o I have both already in place.

o I do not have either one.

o I do not feel I need either one.

o I feel these documents are very important for all persons to have, not just

those with CMT.

o This is a hard subject to talk about with my family.

o I feel these documents are not important.

o I would like to know about how to set up an AD and DPAHC.

o I don’t know what an AD or DPAHC is.

o My family already knows my wishes; I feel I do not have to write it out for

them.

o I have an AD and DPAHC because I do not want the government or courts

getting involved in my personal and private wishes.

o I am uncomfortable talking about this.

o I have added a one page amendment on my CMT to my AD; my DPAHC and doctor

know this.

o My doctor is supportive of my decisions for an AD/DPAHC, accepts and

respects them.

o I have given copies of these important documents to my doctor, and family

members.

o I would like to know websites where I can get information and download free

forms for an AD and DPAHC.

o This is a hard subject to think about and I would rather not.

o My doctor is not supportive of my decisions in this regard and tried to
