nutrition care process

Hello All,

We recently converted to documenting all of our notes in an electronic medical record. The format prior to this did not correlate with the nutrition care process.

I am finding that I am using the same diagnoses with patients and was curious what most of you are using as a dx for post operative patients.

Any suggestions would be greatly appreciated. Thank you!

im am not sure if what i do is correct seeing how im the only RD here LOL

but i use

overweight/obesity r/t excessive intake and decreased activity level AEB

and predicted suboptimal nutrient intake and impaired nutrient utilization r/t ....AEB altered gi functions ....

if they are a r/a d/t not being compliant i use

food and nutrition knowledge deficit r/t ....

and limited adherance to nutrition related recommendations r/t

um i cant wait to see what others are using and if im even close!!!!!
