Hello!

I work with a bariatric surgery program that does about 300-350 surgeries each

year. We currently have 2 FT RD's with the program and are adding a PT RD as

well. Our process currently is that each patient has an initial pre-surgery

nutrition evaluation/assessment with a dietitian, and the pre-surgery education

is done in a group setting and is led by a dietitian. We offer a once/month

6-month class for those whose insurance requires 6 months of weight management

prior to surgery, and a once/week 4-week class for those who do not have

insurance requirements for weight management. In the 6-month class, we provide

education about surgery-specific nutrition recommendations, as well as general

healthy eating information, exercise recommendations, and behavior

change/modification recommendations. Patients complete food & exercise records

monthly, and we provide individualized feedback based on those records.

Currently, our post-op follow up appointments are done individually as well. We

have discussed the possibility of doing group follow-up sessions, however I

cannot fathom how that would work as it does not allow each patient to discuss

his/her personal experiences and difficulties with the post-op diet.

I am curious to hear what other programs are doing with individual vs. group

sessions for appointments.

We do the insurance-mandated preop weight loss classes as a group. We have

anywhere from 10-30 patients in a class so that does dictate the

teaching/discussion style. We offer the class 5x/month (same topic all month)

and patients do not need to register in advance. Each patient fills out a form

reporting their diet and exercise changes over the past month and we weigh

patients at each class so we can document this in their medical record.

We do nutrition assessments and follow-ups individually. The pre-surgery diet

education (diet stages post-op, pre-op diet, etc.) is done in a group.
