I am not sure about the protocol for drugs and alcohol but our psychologist does have a rule that the patient be suicide attempt free and have no admissions to a mental health facility within the past 5 years in order to qualify for surgery. He has also started using the rule that if the patient is on Medicaid for psych reasons that is an automatic "no"

Our psychologist requires 1 year from a suicide attempt or mental health hospitalization before she will allow surgery. If a patient is on disability for psychiatricreasons, she requires they are having ongoing therapy and they have a clearance letter from their therapist.
