When I'm trying to solve such a problem I often start here: http://www.atia.org/ Also, Google leads me to all kinds of interesting sights. As for mounting and operating a camera, TASH http://www.tashinc.com/ is innovative and expensive. If I don't have the funds, I look at their products and drag a geek friend to a salvage yard or the hardware store. There are also many photographers and filmmakers with disabilities around the world.
