Presented by Lyke, Ohio Coalition for the Education of Children with

Disabilities (OCECD)

Participants will gain an understanding of the importance of transition planning

for youth with special needs. Discussion will focus on the requirements under

IDEA 2004 for writing a transition plan, when the process should begin (in Ohio

age 14 or earlier if team agrees), timelines, and who is involved in writing the

transition plan. Methods that will encourage youth to express their interests

and future plans are presented with the use of transition cards.

Each Participant will receive additional handouts that include a set of OCECD's

helpful color coded transition cards (approx. 184) which contain these

categories:

•Employability Skills

•Living Skills (Finances)

•Living Skills (Food)

•Living Skills (Taking Care Of Your Home)

•Living Skills (Safety & Health)

•Living Skills (Self Care)

•Community (What You Like To Do)

•Taking Part In The Community (Transportation)

•Community (Relationships & Social Life)

These cards are used to help the team focus on the specific needs of your child

when discussing a successful transition from school to the community, secondary

education or work.

Take care,
