Thanks Lottie for the update.  I just received my check the other day I always

try to aim for my birthday to get alittle extra .  I appreciate the efforts the

society has come up with during hard times.

Hi Peggy,

 

Try http://lls.org. Click on Forms. I hope this works for you, if not let me

know and I'll try it again. I got the form from my doctor's office. Have you

asked them? Your doctor has to sign it now. It used to be that a PA could sign,

but not anymore.
