New Regimen for long term survivor ??

Hey group, a friend of mine (shy type) has been recommended by sharp doc in SF to try a new regimen. WE WOULD LIKE YOUR REACTIONS, EXPERIENCES, THOTS.. here is the history (times are approximate):

60 years old, deals well with side effects

hiv probably 25 years or so.

currently on Combivir alone, for last 8 years

genotype indicates virus has mutated just about everywhere (only firm note is sustiva class resistant)

counts for last two years:

T4 T8 VL

(all below 10k until '04)

Doc is recommending the following (we aren't sure if doc wants 3 or all 4):

We can construct a new regimen, using the following medications:

1)TRUVADA (tenofovir and emtricitabine) one pill once a day

2)Nevirapine 200 MG once a day for 2 weeks and then one pill twice a day

3)Darunavir (PREZISTA) with 2 pills + one NORVIR capsule TWICE A DAY

4)RALTEGRAVIR (Isentress) one pill once a day.

You cannot take the maraviroc (Selzentry) because your virus is not CCR5 tropic.>>Start the year off right. Easy ways to stay in shape in the new year.

Has her CD4 cell percent gone down? She has very high CD4 cells and low viral load to go on such an advanced regimen. Without seeing her geno and phenotype, it is hard to tell why your doc chose such a salvage regimen. By the way, raltegravir should be taken twice a day.

If she has been on Combivir alone for 8 years and you are doing that well, something tells me she should not jump to such an advanced salvage regimen. But I am sure there are reasons that you have not told us since this does not make much sense to me. Regards,

Hi Eddie

If your friend has NNRTI class resistance then they should not be using

nevirapine.

They shouldn't use nevirapine anyway because they have a detectable CD4 count

way over 450 and do not have an undetectable viral load.

A combination with the remaining drugs would be likely to work on most people,

even if they had broad resistance to PIs, NNRTIs and nukes - though this also

depends on the detail of their resistance tests from bloods sample taken on

previous failing regimens. Some rare mutations may make tenofovir unlikely to

work, and even darunavir - and if they do have this level of resistance, then

using T-20 would be likely to work.
