Vaccination Exemption Letter - State of Ohio

Kerri, I live in OH and my 2 children(1ASD 1 NT) go to public school and have not been vaccinated since my son's reaction in 1996. When you tell them you are not vaccinating the school nurse will give you a paper to sign....that's it end of story. It is very easy. is not required to be immunized.

Just write a note with the wording of the above - ie............ " I

decline to have ____________ immunized for reasons of conscience "
